I suppose that Death has given a valid answer, as most people would stop playing a piano just before they dropped dead (from poisoning).

But if he died because the music stopped...

He was playing the Wedding March on the organ at a wedding service and as the bride was half way up the isle he stopped to answer his mobile phone. The groom, who was a very, very angry man, in a fit of uncontrolled rage, exacted the ultimate revenge on the organist for ruining his beloved's day.

A crocodile ate a music box. As long as the music box was playing, Captain Hook managed to escape the crocodile. But after a couple of years, when the spring was completely unwound, it stopped playing and the crocodile could get the unsuspecting Captain by surprise.

Perhaps the man faced El Indio in a duel and as soon as the pocket watch finished its eerie melody he reached for his gun, but in the blink of an eye Indio drew his gun and made the fatal shot.

Crater? Bit exagerative, my answer is he was in a mob HQ. Was asked to play as he was the best in the area, did not wish to continue playing after they suggested he keep going after playing for 18 hours straight. When he stopped they shot him.
