An airliner was operated by a two-person crew. Captain Smith was on board, in topnotch health, incapable of pilot error and with a perfect record for flying airliners. The airliner was approaching a well illuminated airport in order to land, but crashing due to pilot error. The first officer at the co-pilot's wheel made no error. Can you explain it?
