Several reasons :
1. Timothy himself might be holding the ace of hearts, so he knew it's impossible for Damien to hold another one. ( unless they're playing a game which requires two decks )
2. Timothy thought Damien was referring to his personal life, and given than Damien just broke off with Timothy's sister, it's hard to accept his claim of being a nice guy.

Yes. Upside down or not the leaf has the shape of a heart.
